HUD-VASH Social Worker Case Manager

Some of the duties assigned include, but are not limited to:
The incumbent is responsible for program oversight, monitoring and administration, serving as the primary contact with local Public Housing Agencies (PHAs) and providing case management-based services (directly or through referral) necessary to ensure that veteran care is coordinated with the VA medical center, community health or mental health providers, and agencies that serve homeless populations. The incumbent provides social work services at a field location alone or with subordinate social work staff members without a supervisor available for consultation. Field locations may include, community-based outpatient clinics (CBOCs), satellite outpatient clinics, participant residences, or other community locations. The highly complex needs of the homeless/at-risk of homeless population served by the incumbent will require that the incumbent have expert level skill in provision of psychosocial interventions. Such assignments typically require extensive coordination of services with a wide range of residents of various communities such as local lawyers, physicians, and public officials, and with local social agencies. The incumbent works directly with veterans within various community housing settings and community sites and provides supportive case management services and partners with community agencies to link access to community-wide housing services for veterans. The incumbent provides services such as psychosocial assessments, treatment planning, individual, group, and family therapy, discharge planning and follow-up, crisis intervention, case management assistance, resource development, and coordination related to these services as clinically indicated. The incumbent will provide case management and referral assistance to the veterans that assist consumers in meeting their goals and objectives within a psychosocial rehabilitation framework that allows successful reintegration into community living. The incumbent makes independent professional decisions and recommendations with infrequent recourse to supervisory guidance. The incumbent provides direct social work, mental health, and substance abuse services for veterans and family members or significant others in support of the veteran's treatment. The incumbent assumes responsibility for independent professional functioning and regularly works with a variety of complex cases within the assigned areas. Work Schedule:

Have a master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure:
Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website. English Language Proficiency:
Social workers must be proficient in spoken and written English. Experience and/or Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in administration, demonstrating progressively more professional competency skills and judgment. The candidate may have a certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
. (a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. (b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. (c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. (d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. (e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred

Senior Social Workers have experience that demonstrates possession of advanced practice skills and judgment. At least two years should be in an area of specialized social work practice. Senior social workers must be licensed or certified by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice). Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Must pass a DOT physical to be considered for this position. Light lifting (under 15 pounds), straight pulling, reaching above shoulders, use of fingers, walking, standing, near vision correctable at 13 to 16, Far vision correctable in one eye to 20/20 and to 20/40 in the other, hearing (aid permitted), emotional/mental stability and read written material. The Social Worker usually spends most of their time in an office, but they also may travel locally to visit patients, meet with service providers, or attend meetings, and involves risks with exposure to potentially dangerous situations or unusual environmental stress.
